Summer Camp medical forms

All scouts going to summer camp must provide two copies of a completed BSA Medical form - Parts A, B & C. Part C must be completed by certified and licensed physicians (MD, DO), nurse practitioners, or physician assistants.  You must attached a photocopy of your medical insurance card, or note on the form if you do not have medical insurance.

This is a requirement for all BSA Summer Camps and no exceptions are made.  Part C must be signed by a doctor and dated within one year of the date you leave summer camp, so for Troop 137 this year, your medical form must be signed after July 23rd 2016.  That means if you visit your doctor any time between now and summer camp, you should have them sign the form.  

There will be an evening set aside about 1 week prior to summer camp where we will ask all parents to stop by the meeting room and drop off permission slips and medical forms.
